I watched the episode of Air Crash Investigations (or something similar) about this episode. One girl said she noticed a small crack coming off from under one of the rivets as she was boarding. Basically, the plane was used for those comparatively small jumps between the islands which means they haven't exceeded their miles alotted for the air frame but the cabin is still pressurized and depressurized with each flight; putting lots of stress on the airframe. Likely what caused this catastrophe.

....they told me an oxygen mask would drop down....from the overhead.....where was my mask.....I didn't see my mask......I was gasping....but no mask.....

I've watching lots of these Air Crash Investigation and Mayday documentaries..Some of these pilots are incredible bringing doomed aircraft with crew and passengers back to ground and safety.

I’m hoping that poor woman was knocked out by the force of the wind or by the lack of oxygen at that elevation. Either way, it’s still horrifying!

There was blood splatter found outside the fuselage where the hole was and the assumption is she hit her head as she was pulled out so she likely died instantly.
