On March 3, 2023, a Bombardier BD-100-1A10, N300ER, operated by Conexon LLC, was involved in an accident near Windsor Locks, Connecticut. The two-engine airplane sustained minor damage.

A passenger whose seatbelt was not fastened was fatally injured.

The accident was caused by the flight crew failing to remove the right-side pitot probe cover before flight, departing with a No-Go advisory message after an aborted takeoff, and selecting the wrong non-normal checklist in flight. This led to an in-flight upset that exceeded the airplane's maneuvering load factor limitations.

The severity of the upset was increased by the pilot-in-command continuing the climb and using the autopilot while troubleshooting, followed by pilot-induced oscillations after the autopilot disconnected from an out-of-trim condition. Inadequate crew resource management also contributed to the accident.
