AirAsia's Severe Engine Failure Violently Shakes Plane For Two Hours

Perth, Western Australia, June 25, 2017 – An AirAsia X Airbus A330-343X, registered as 9M-XXE, encountered a severe engine failure mid-flight, leading to intense vibrations reminiscent of a "washing machine" for the duration of its return journey to Perth. The Australian Transport Safety Bureau (ATSB) has completed its investigation into this alarming incident.

The flight, originally bound for Kuala Lumpur, Malaysia, was approximately one hour into its journey when the crew heard a metallic bang, followed by significant vibrations throughout the aircraft. The left engine (ENG 1) had stalled, prompting the flight crew to execute an engine failure procedure and return to Perth on a single engine.

Despite attempts to mitigate the vibrations by altering altitude and airspeed, the aircraft continued to shake for the remainder of the flight. The Airbus A330 landed safely in Perth, where it was discovered that a substantial portion of one fan blade from the left engine was missing.

The ATSB's investigation revealed that the fan blade failure was due to fatigue cracking within a high-stress region of the blade. This cracking was attributed to a small acute corner radius at the junction of the internal reinforcing membrane and the convex skin panel of the blade, a byproduct of the manufacturing process. The size of this radius varied due to the complexity of manufacturing, and its potential impact on fatigue cracking was not previously identified by the engine manufacturer, Rolls-Royce.

In response to this and several previous fan blade failures, Rolls-Royce had initiated a non-destructive inspection program designed to detect internal cracking. However, the inspection interval was found to be too long to capture the crack in this particular blade before it reached a critical size.

Why didn't he shut the engine down? I'm surprised the engine computers didn't simply shut it off by themselves. Should have been able to fly without all the commotion......

He probably did, but the engine would keep turning in the slipstream. Something let go, that set up an imbalance, a chunk of a fan blade maybe?

I've paused and re-wound the part of the last video in the OP multiple times. To me it appears as if a mount or internal housing broke which is causing the engine to vibrate a lot more than normal. I'm consulting with a friend who is a jet fighter mechanic to see what his opinion is.
