thats no dislocated knee cap, that dude has snapped his tib and fib ~ you can even see the two "holes" were the bones should be
It's an actual dislocated knee -- very rare (the tibia and fibula are dislocated out of the joint but probably not actually broken).

It gives me the shivers just looking at it!
