Very little about medical procedures in 1910 was sanitary.... And he doesn't have forceps in his mouth... click on the image... it's a pipe.

Smokin' in the morgue room, that's right we were smokin' in the morgue room. Now coroner don't you push me around with your rules cause every dead body knows that smokin' ain't allowed in med school.
