Leg Lost To Vehicle Accident

A young man was brought to the trauma center following a high-speed motor vehicle accident. His upper body was protected, but both feet and lower legs sustained severe trauma.

Note severe disruption of the ankle and syndesmosis. Marked displacement is indicative of severe soft-tissue injury. Marked soft-tissue contusion and disruption over the distal anterior tibia and medial ankle and, most significant, of the weight-bearing skin, muscle, and specialized fatty tissues under the heel. The foot lacked a pulse and was insensate. The zone of injury was notably large.

Medial radiograph of the left foot. Note severe multiple dislocations of both essential and nonessential distal joints in the midfoot and forefoot. The amount of displacement indicates that major neurovascular compromise has likely occurred.

The extent of degloving suggests that the foot is unsalvageable. If salvage was attempted the skin would be markedly necrotic and the foot would have severe problems from edema and neuropathy.

Despite the fact that the patient sustained bilateral injuries, the results of the evaluation indicated that neither foot should be salvaged and immediate below-knee amputations were carried out.

I'm certainly no expert but the foot in X-ray C didn't look to me to be so severe that it called for full amputation below the knee. But like I said, I'm no expert and considering the doctors thought it had to be amputated I guess it did. Sucks to be that guy.
