It seems to me that the cause of death was what made her expel the fetus, that's why the size.
I wonder what killed her...

Judging by the stage of decomp of the mom, the fetus could have been expelled during a phenomenon known as coffin death. The gases produced during decomposition eventually bloat the uterus and the baby is forced out through the birth canal. It looks like the uterus is being expelled too. Horrible isn't it?
