People aboard tried to use the emergency brakes but did not work. Then they managed to widen the opening and release the woman.

The driver failed to spot her because he was eating while driving, he did not stop despite passengers on board pulling the emergency alarm. You must look in the rear-view mirror before starting the train; he's now under investigation on charges of grievous bodily harm.

in the UK there is a guy who checks the train is all clear and then puts up a signal and blasts his whistle......i'm sure stupid shit like this still happens though....
