A Prescott, Arizona, police officer fatally shot a 44-year-old man who charged him while holding a knife in each hand, according to accounts of the November 2013 shooting later reviewed by prosecutors.

Officer Jared Willis responded to a home in the 4800 block of Hornet Drive on Nov. 19, 2013, after a 911 call reporting that a man inside, identified as Larry Clay, was suicidal and had been drinking, authorities said.

Willis told investigators Clay ignored repeated commands to drop the two knives he was holding, then advanced on the officer with the blades raised. Willis fired three times, and Clay died at the scene.

The Arizona Department of Public Safety investigated the shooting, producing a report of nearly 300 pages on the incident and Clay's background.

In May 2014, the Yavapai County Attorney's Office said it found no evidence Willis had acted improperly and declined to file charges. Prosecutors wrote that Clay's actions were consistent with what investigators call "suicide by cop," in which a person provokes officers into using deadly force.

Recordings from the response, including 911 calls from Clay's relatives and Prescott police radio traffic, were later preserved as part of a public archive of emergency dispatch audio from the case.
