At about 1 p.m. on April 3, 2022, Chicago police officers Ivan R. Lopez and Martin R. Kirkel were sent to a multi-unit residential building near 4200 W. Ford City Dr. after an OEMC radio broadcast for a person shot. Inside an apartment, they found a woman prone on the front-room floor. She told them a man had shot her, had also shot her father, and that both the man and her father were still in the apartment.

When the officers called out to the man, he fired two shots, apparently in their direction. Neither officer was hit, and neither returned fire. The officers retreated and, with Officer Joseph J. Shanahan and Sergeant Daniel J. Hodges, evacuated the woman.

Officers secured the apartment, and Officer Lopez remained in the second-floor hallway outside the doorway for about 37 minutes, telling the man to drop the weapon, show his hands, and submit to arrest. At about 2 p.m., Officer Orlando Sanchez Jr., a Chicago police SWAT officer, took over communications from the doorway while other SWAT officers stood nearby.

For about 1 hour and 13 minutes, Officer Sanchez tried to get the man to surrender. During the exchange, the man made statements indicating suicidal ideation. At about 3 p.m., immediately after one of those statements, Officer Sanchez said, "Don't do that. Put the gun down." Seconds later, he fired five times into the apartment.

Officer Sanchez later said the man raised and pointed a handgun at him before he fired. Other SWAT team members said they could not see the man at that moment. Officers then entered the apartment and found the man prone and bleeding on the front-room floor next to a revolver.

Officers rendered aid to the man and to the woman's father, who also had one or more gunshot wounds. The man died from gunshot wounds to the front of his body. The woman and her father survived.
