Brian Diez, 26, fatally shot five people at a Lake Havasu City, Arizona, home just before midnight on Aug. 28, 2010, then drove away with his two children before officers found him dead in California, police said.

The 911 caller was Deborah Nyland, 44, who had been shot in the neck and was the only person inside the house to survive. She told a dispatcher an intruder had walked in and shot several people, and said she was not sure whether the others were still breathing, according to The Associated Press.

Police identified the gunman as Diez, who had two children with his estranged former girlfriend, Deborah Langstaff, 23. Langstaff and several friends were at the home on Opossum Drive celebrating the birthday of her boyfriend, Primo Verdone, 24, when Diez came in with a handgun, authorities said.

Langstaff, Verdone and Ashley Nyland, 20, were pronounced dead at the scene, along with Ashley Nyland's father, Russell Nyland, 42. Her boyfriend, Brock Kelson, 20, died of his wounds early the next morning at Havasu Regional Medical Center. Deborah Nyland, Ashley's mother, was flown to a hospital in Las Vegas and was listed in good condition by the following Tuesday.

Diez left with the two children he had with Langstaff, 4-year-old Kaia and 13-month-old Cole, and drove roughly 250 miles to Rancho Cucamonga, California, police said. He left them unharmed with relatives there. Officers found him dead of a self-inflicted gunshot wound at a house in Rancho Cucamonga about 3:30 a.m. Sunday.

Langstaff, a nurse at a local hospital, had taken out a protective order against Diez earlier that year after accusing him of slapping their infant son, court records showed. Diez was arrested Aug. 13 on an allegation that he violated the order by calling and texting her, and he was facing a separate theft charge over a vehicle he was accused of selling twice on eBay. Langstaff had also gone to court to establish paternity and seek custody and child support, and Diez never responded to the petition. Investigators said a motive would be hard to establish because everyone in the house had been killed or hospitalized. Lake Havasu City police called it the "worst shooting in the city's history."
