On February 8, 2023, around 3 a.m., a 911 call reported that a man had pointed a handgun at a security guard for Christina's Place, a bar at 3759 N Kedzie.

Chicago police Officers Jose Carcamo and Jorge Ulloa responded and encountered the man with a gun in his hand. The officers pursued him on foot.

The pursuit ended in the alley behind 3753 N Troy, where both officers discharged their firearms, fatally wounding the man. The incident was captured on body-worn camera and third-party surveillance video.

Administrative allegations later stated that Officers Jose Carcamo and Jorge Ulloa discharged their firearms at or in the direction of the man in violation of G03-02, failed to timely activate their body-worn cameras, failed to notify OEMC of a foot pursuit, and failed to render medical aid. Findings were sustained and not sustained.
