When I worked at Bombardier, we had a fuel truck driver at our Canadian operation, who died like that, that because of a freak accident. They wired the doors into the emergency brakes, so if any of the equipment doors on the fuel tanker fell open, the emer brakes went on. Unfortunately, this "safety" feature functioned exactly as designed when an equipment door opened after he hit a bump at 50 miles per hour. The brakes screamed on, and he flew right through the windshield and landed 50 feet in front of the truck, stone cold dead. Having to get in and out of the truck every other minute, he wasn't in the habit of using his seat belt in the truck, although his friends said he did use belts when driving his own car. All the fuel trucks in our entire network were parked for a few days until they straightened that feature out so it wouldn't kill any more employees. |
