She reportedly died. Maybe she got used to hearing that beeping sound all day long so it loses importance. When i drove a forklift a long time ago i had two mirrors and always looked into the direction where the forklift was going.

It could just be complacency. At my work there's always forklifts in the warehouse and the constant beeping can fade into the background. I still keep my head on a swivel thanks to videos like these though. I really do think these should air on public TV with no censorship at all. It's something that's talked about but nobody really knows just how bad things can get and they're never prepared for it.
