looks like the driver was only focussed on the light to turn green while the pedestrians didn't walk fast enough when their light was flashing orange/yellow

I didn't see that one coming. I was concentrating on the oncoming traffic. This was as surprising as a weiner popping out of the bottom of your date's popcorn bucket.
