It all depends on the context. "No mames" could mean "you're full of shit" or "you're lying" but also could mean "for real?" or "are you serious?"

Anesthesia was only invented in the 19th century. Before that they gave the patient alcohol to get him/her a bit drunk and something firm but pliant between the teeth to bite down on (The expression "bite the [lead] bullet" comes from this). Battlefield amputations were commonly done this way. The stories of Civil War surgeons are not for those with weak stomachs.
