Maybe you are supposed to clean it without inserting limbs and this guy wasn't following proper procedures? In the good old days it was your day in the barrel and you just got fucked. This happens so often. I would have a machine like that plugged OUT or have the shut off switch locked in case a dumbass thinks it's off for no reason in the middle of the day.
