Watching this video, I can see two things: one, men working side by side with heavy machinery. Two, there is no barrier between the men and the pit. No safety device preventing them from falling inside the Garbage Disposal.
You MUST HAVE safety devices when you have high probability of serious injuries. Trusting purely on attention is one of the big mistakes I see being made inside companies. Humans are humans, and they WILL fail. That's for sure.
Now, I hope that company be forced to pay a really heavy indemnification. But there is no price to be paid, when you get such a tragedy which could be easily avoided.

It helps you get visualized by others (because those special clothes always had some iridescent stripes). The flashlight would be useful to see how close you actually are from the pit. We can see from the video how dark their workplace is.
