I'm wondering if he was performing maintenance on the machine and it wasn't "locked out". I worked in Factory Automation for 30 years and I've heard a LOT of maintenance accident stories for sure.

Yes, and many of the presses require a "palm station" to cycle. There are two buttons spaced far enough apart to prevent one-handed operation. I'd guess this guy was maintenance/tooling/setup. From my experience, those guys are too cool for LOTO procedures. |
