I don't think she tripped on anything. I think the crappy uneven pavement (as we can see in the pictures) caused her take a misstep and fall. People fall when there's an unexpected level change on the ground - don't necessarily have to trip on anything.

That was intentional. She propelled herself at the last moment and her head was directly where it would be crushed. If she had fallen naturally, she wouldn't have been under the bus.
