It was twins at least, but judging from the gestational age, most likely triplets, that allowed the birth of one of the babies. The second one was stuck in the birth canal, and the sac still intact. The baby in the toilet had the full placenta still attached. The most likely explanation for the mom dying was from blood loss internally because the placental attachment in the uterus would have bled copiously. There could have been a placental abruption, or an amniotic fluid embolism as well. It is also possible she was suffering from eclampsia, which is common in multiple pregnancies, and causes stroke and death in pregnant women.

looking at it again, you can clearly see the head of the second baby at 00:22 to 00:30. The uterus clamps down immediately after a baby is born, and likely there was a shoulder dystocia.

i think the music is supposed to be sad but the tone and the beat dont create the emotional effect they want.. screeching like that hurts the ears.
