Substandard cheap bargain basement discount hardware.

They probably saved $50...at most.

They could have easily created a rig that could have secured him safely for several non-stop circumnavigations of the globe.

I have some straps that are rated at 10,000+ pounds that were cheap enough.

This was a Death Wish being granted.

His belt didn't break, the steel ring to his harness did. Stainless steel rock climbing hooks are not designed for stress under g-forces, ( he exceeded the weight limit when the plane turned ).
No offense, but that statement is completely wrong. Even an aluminum carabiner could have held him easily under those conditions, let alone a steel one. Whatever failed, it wasn't a climbing carabiner.
