There's at least one documented case of a person opening their eyes in response to their name being said after dog's blood had been pumped through their severed head. Whether it was reflex or actual awareness, it does imply consciousness of some kind.

Wow, that's some video! As mentioned, there have been a few experiments and direct observations, in particular around executions via guillotine. I've always been fascinated with how long one stays aware after decapitation. In doing some research on the matter, I read a story about a dude that was in a horrific car accident with a friend. The friend was decapitated. He reported that when he looked down at his friend's head, his eyes looked from him, to his own body and back, with sort of a look of realization of what had happened before he faded away. Who knows if that's a true story, but this eye movement seems to support awareness for at least 7-10 seconds.
