The two main issues are loss of oxygen to the brain, and loss of blood pressure.

The brain has a decent 10-15 seconds worth of oxygen to keep you functioning somewhat "normally" mentally before it starts to fall apart. Not much you can do about that.

The bigger issue is rapid loss of blood pressure and losing consciousness and the general shock of it all, etc.

If you *really* wanted to be a dick, you'd rig something up where it beheads the person but locks in place. Like, a horizontal guillotine where the blade slices their neck clean but then just sits there locked in place and prevents blood loss.

I'm pretty positive that this would allow for a pretty disturbing and lengthy period of true consciousness.

Yes, in fact some crude experimentation has been done on this during the French Revolution, after noticing that several of the heads seemed to have some reaction after beheading. One in particular was a man whose head had been picked up and slapped across the cheek, his face showed the shock, eyes gone wide in disbelief, and his cheeks flushed, albeit briefly.
There is a documented case of a scientist who was set for beheading. He asked a colleague for help in a final experiment, he would blink his eyes for as long as possible after decapitation.
It’s reported that he blinked 30 seconds.
So there is certainly ample evidence to at least suggest that there is some level of conscious awareness after the head leaves body, and before the oxygenated blood leaves the brain.

Holy shit, one second you're happily riding your bike, the next your head is severed off. If he had looked closely in the rear view mirror maybe he could have put his head down and prevented it.
