You know it's bad... very very bad... when the accident is so enormous, the injury so severe that the person becomes an individual of indeterminate gender in the video

This reminded me of a scene in a very old silent film I once saw where a kid working in a factory gets pulled into a machine and gets the fatal spin treatment. It was surprisingly graphic for a film from the 1920s, especially when the father is crying over his son'e dead, mangled body.
Colour me intrigued, got the title?
