Trains can sneak up and suck you off!
On the platforms here (suburban Chicago) they have a yellow line painted several feet from the edge. Trains here can go 75+ mph and they create a wind or suction that has pulled in people and killed them. So the yellow line is where you shouldn't go lest a train sneaks up and sucks you off (the platform).
In this case something was hanging off the train, maybe a board or steel beam. In that part of the world they probably aren't careful about safe loads and if they loose a bit of cargo in tunnels or under bridges or hitting buildings, vehicles or pedestrians, no big deal....
