looks like the shot came out higher as the entry point and the subclavian artery seems to be a rather big artery as blood was squirting out at the entry wound straight away.

My best guess is that it punctured the brachial artery running across your collarbone and down your arms. It's one of the five major arteries in the body Edit; my bad. Left subclavian artery as Kellyhound pointed out above me |

Bellets tend to change trajectory when impacting bone(depending on calibre of course), in that area of the body there are many main arteries. Bullet may have shifted and severed the artery in the armpit. bleed out in seconds.

As a mother I definitely would have instantly covered my child's eyes or picked her up and ran. I have no idea what kind of fight or flight this was but not the typical response of a parent when their child is potentially in harms way

The angle of the shot, probably entered his heart and out his shoulder. If not, there's a major artery that runs through there.
