I'm going to guess he was involved in an auto accident. It looks like he's only wearing socks and I don't see any obvious signs of trauma besides the blood by his head. I think maybe it's a closed head injury from being hit by a car.

I think its 'just' a epilepsy attack.
I do not see marks from a incident and he has all his cloths and shoes on his body. People with Epilepsy can have certain attacks, and often fall with the head first.
