Na, 1st and 2nd year med students typically aren't allowed to have any physical contact with patients. Most of that time is spent in lecture halls and libraries, attempting to learn an ungodly amount science in a very short time... so taking that into account, theses dudes are most likely in their 3rd year.

However, the exception would be if the med school they attended was based in the Caribbean, in which case all these men are actually doctors.

(for the uninitiated, med schools based in the Caribbean are known to have very low standards for admittance. For example, say you've spent the last 10 years of your life totally dedicated to your career, but you feel like you've gone as far as you could go and decide you're ready for an exciting new career. You could apply to one of their schools (doesn't matter which, you'll be accepted immediately) and then you can look back on your 10 years of being a Wal-Mart greeter with confidence, knowing you're fully prepared for life as a Caribbean med student.)
