I cold water dip and swim, you don’t bomb or throw yourself in, you always have an exit route or strategy, you always use a tow float (with a torch in at night or glow sticks attached to you) you never trust god to save your dumb ass from a freezing flowing river in the dark  |

It was obviously too late for the rope, but putting a strong light (headlamp?) immediately down the hole might have given her this 1% chance of survival.  It might be, that at the same time when they were panicking above the ice, she was panicking beneath trying to find the way back. What an awful way to go... |
