A famous photographer by the name of Timothy Panasiuc (Motif) was killed in an accident in the Narofominsk region of Moscow.

He was riding between two lanes when he collided with a car and fell under the wheels of a truck.
He died at the scene.

Didn't quite look like he was riding between lanes, just trying to move over to avoid a collision, only to find he had nowhere to go. Looks like he might not have been quite tall enough for the bike he was riding, and as his foot touched the ground, it slid and then his knee buckled, causing the bike to go down hard. I'm about 6'3" (about 190cm for you blokes across the pond) and my Harley sits down low enough that my wife who is a foot (30cm) shorter could comfortably have both heels on the ground and her knees bent.

Another thing that I do that I feel helps keep me as safe as possible, is to ride directly behind the driver of the car ahead of me. I stay in the lane closest to the inside shoulder, giving me a good escape route should something like this happen.
