There's no evidence from what they say that this was done inside the hospital. From the tone of the examining doctor, this man must have been an addict or a vagabond. The doctor barks a couple times for the guy to turn his head and sounds like he's annoyed that he has the burden to tend to this guy. He's disrespectful. I'm guessing this was a drug-related crime, because Iranians are very contemptuous of drug addicts and poor people.

According to what the doctor said the guy's trachea is completely destroyed and his esophagus is ruptured.
