The bullet probably went through his sinus cavity. When he's "upside down" you can see the blood start oozing from the nasal passages at the roof of his mouth.  |

Notice the muscular action in his throat - with the blood coming from the nasal cavity, the blood will come out anywhere that connects, but still his orbits are swelling too. Close perfect ill shot that is a bit high - what I find interesting is that the brain is swelling top down - it's not quite pressuring through the brain stem and I think his gag is functioning
