can someone please explain to me if during agonal breathing they are consciously gasping for air, or is it more of a automatic survival mechanism/reflex? I could google it but would rather get the answer from the great folks at DR
Well, the short version is, no one can really explain it, except to say that it is a reflexive thing brought about by the brain stem in the last few minutes of life. It isn't really breathing either, as there is no chest rise associated with it. At least not from what I have seen from my own experience.

I've got a really good example for you. Several years ago I responded to a respiratory distress call for a female in her mid-fifties. Things quickly turned bad, and she coded on me, going into full cardiac arrest. I watched her go from fully alert to completely unresponsive in a matter of about one to two minutes.

Our protocols call for us to use the exact same equipment, medications and CPR techniques as they would use in the hospital, and if unsuccessful, cease life saving efforts after thirty minutes. If signs of life are present, but we are not able to obtain return of spontaneous circulation (ROSC) we will begin transport to the hospital.

In this particular case, that is exactly what happened. After about ten minutes, I got a heartbeat. Then lost it. Got it back, then lost it again. We worked this lady for just over an hour, from her living room floor, all the way to the ER. Almost from the very beginning, we saw agonal breathing. We were hoping that it was signs of life, but sadly, it was not. Even after we stopped CPR at the hospital, she was still doing the same thing. The only thing I could attribute this to would be the fact that high quality CPR will deliver oxygen to the brain, keeping it going after the rest of the body has shut down.
