I've seen something like this in hospital before. A guy killed his wife and a man she was with and then put a gun to his chin. Basically the same sort of wound occurred with the bottom half of his face blown off and the bullet exiting through the bridge of his nose. The doctors reconstructed his jaw bone and some facial structures by using other bones in his body and then patched up the outside with skin grafts. Over a year later, after he healed up, he didn't even look human.
