Most likely he was catapulted from a motor bike of some sort and the force of it wedged his head in the truck. I've seen similar things in head-ons involving trucks and bikes. |

My guess is that it is a dump truck of some sort and he was poking around trying to fix something while the box was up and it and the valve in the cylinder failed. Ka-Slam... pancake head. Biggest no no in the industry, block it up before working on it.
