Don't these machines have any safety mechanisms? Every press I ever used in the shit factory I worked in had two buttons you had to press at the same time, so that no limb is in danger.

I think it was probably pretty clear that she was dead as a door knob. He was in shock, why on earth would his instinct to be to try and scrape her crushed remains off of the machine |

I'm a risk manager and I've investigated more than a few "lock out / tag out" related death claims. Some just didn't bother to tag ANYTHING, but most often occurs when systems that control the injuring mechanism are located out of site. Sometimes in another building entirely. "Tag fan three-bravo-six." Someone tags three CHARLIE six. Guy goes into the fan bay to repair the fan, bang comes on. In the U.S. OSHA is involved in every death or serious injury and they try to improve the systems that govern the way machinery is locked or tagged out, but mistakes are made. Please be careful out there, brother, every day. |
