government doesnt do much in way of caution for drivers and vehicles, pedestrians and drivers refuse to use what little facilities provided by government if they have to go out of their way ( overhead footbridges for pedestrians, who are usually placed a block or two from where many people used to cross... or drivers refuse to obey one way traffic if it would mean doing a u-turn on the block ahead)...

so mostly because government failed to provide proper caution material or the people just dont care to obey because they too stubborn and bullheaded.
