You can see where the pickup (eye-bolt) failed. It was probably worn through. A simple visual safety inspection would have prevented this.

Dumb M/Fucker. He reached up to grab it to try & guide it into place on the truck bed when in fact he should have been using a guide rope from the ground. A guide rope is tethered to the slug load to as a mechanism to stop the load from pivoting & swaying around on windy days & always used regardless to help guide or land the load into its resting place on the ground or in this case onto a truck for shipment. This person shouldn't have been on that truck at all, he should have been on the ground holding onto the guide rope & assisting in stabilizing & lining up the load for it to be lowered into its final resting place to then be secured onto the truck bed with chains ect before transport. Complete fuck up all round with no decent standard operational procedures in place. Riggers & Doggers Courses have basic safety guidelines for the management of a load being shifted & a rope guideline is No 1 on the safety side of things & considering China isn't a 3rd world country, they should know the basic principles of load shifting safety. I would say that this company will end up in a huge hole because of this, even to the point of manslaughter charges being bought against them as they are ultimately responsible for their staffs training & standard operating procedures ect & because of its failure to enforce these SOP's, that this person's family will end up getting millions for their loss & the higher up's will be in for a hell of a ass kicking, & so they should as this fella lost his life for lazy site management, nothing more. He shouldn't have been on that truck while the lift was being carried out ! |
