Looks like foundation failed as a result of the tower fall, not the other way around. My theory is the crane was placing the tower on the foundation. It looks a ground worker started the first nut when the tower separated and toppled. The point load of the tower heel might have cracked the concrete, but the tipping flange grabbing the lone anchor bolt no doubt cracked concrete too.
