Usually its due to pulmonary aspiration, which is basically when you swallow something (in this case, blood) and it accidently goes down the wrong tube and into your lungs and you choke and cough it back up. When you get shot in the face you can lose your ability to swallow properly either because the part of your brain that controls the swallowing reflex is fucked or your tounge was damaged. But the appearance could also just be bits of brain, or stomach contents.
