I am guessing there's a pretty good chance that he actually cut her throat for her. I get the impression its pretty hard for someone to successfully slit their own throat.

What's also interesting is that I'm pretty sure you can see that he pretty much got used to and comfortable with breathing through his neck. Looks like the trick is to take lots of little breaths so as not to suck a shitload of blood in.
