My guess would be a short + the fan looked to be all metal so the short could have conducted through the frame. That or the switch had a short. Either way something sent current where it was not suppose to be.

Its unlikely to happen in first world countries. Appliances need to pass tests and be properly grounded in Caucasian and honorary Aryan (Japanese) countries (ooops!). Electrical outlets are properly grounded also.
