2 Oil Well Drilling Rig Employees on Fire and Die After Explosion
2018, a process engineer for a company that performed repairs on an oil well drilling rig in Russia failed to measure the air/gas environment and instructed the repair team to continue working. When there was an ejection of gas-oil mixture followed by a firing blast 2 employees died while a third one suffered serious health problems. The engineer was found guilty of violating industrial safety requirements and a court sentenced him to 2 years of imprisonment. |

They almost started nuclear war by accident in 1983 when their shitty faulty equipment indicated a full scale US first strike IIRC.
