Fake, there should be much more blood, esp. dripping from the end of the needle. Also the entry area looks like too much pressure (even blanched!) when compared to the other entry.

The small quantity of blood doesn't surprise me. But I still think it's fake because it looks like it's going too deep through the heel. There isn't quite that much fat padding there. Point of entry also looks poorly-defined.

iono if its fake, i mean it matches up and there isnt alot of blood which could be normal depending on blood flow and conditions, also the placement of the needle relative to the foot. i mean it looks deep into the heel which i bet it is, but her/his foot is tied to the bed so i dont think its meant to be enjoyed. maybe that guys a super asshole. =/ but iono my feet are fine so idc if its a fake or not.
