From my little research on this subject it is the liquids they are using to break down the Tablets and all of these people who have limbs falling off is poorly due to missing and not emptying the full syringe in the vein. Even so with the amount they sometimes use its gonna fuck them up!

The tablets contain silica compounds that do not dissolve and must be filtered out of the solution prior to injection, otherwise the insoluble crystals will block capillaries, and form blood clots, cutting off the blood supply to cells. Then, the cells die and autolysis occurs.

this is desomorphine also called crocodile, because it obviously "eats" the flesh of it's users. Junkies fom Russia produce this from codein pills because it is cheaper than heroin.
