Terrible case of transmetacarpal amputation after a heavy door closed over the patient's hand. Unfortunately, the fingers could not be rebuilt due to the extent of the damage.

Successful replantation and revascularization at the transmetacarpal level generally results in fair to poor functional outcomes, even in guillotine-type amputations.

The main reason is probably ischemic damage to the interosseous and lumbric muscles as a result of direct muscle injury or disruption of the delicate blood supply to these tiny muscles.

The zone of injury is often extensive and destroys the intrinsic muscles, flexor and extensor tendons, MCP-Joints, and the neurovascular structures. Multiple reconstructive procedures are required in a graded fashion.

In general, complete hand replantations have a better prognosis than transmetacarpal replantations.
