Cyclopia, or holoprosencephaly. Awesome picture. Caused by a failure of the brain to separate into left and right hemispheres. The same process that separates the forebrain also moves the optic nerves and the eyes out to their correct location and causes the cells that will become the nose to migrate downward. For reference, these babies don't survive.
