i figure the pen got lodged into the eye itself with the eyeball moving aside and the pen going under the nearby bone and skin without leaving any external mark. The child likely noticed the end of the pen sticking out of its eye/head and yanked on it leaving the tip imbedded under the skin -- anyway that's how i figured it to make sense.
