Superior Vena Cava Syndrome

Stokes collar

Swelling of the neck, head, upper limbs, and shoulder blade area occurs as a result of compression of the superior vena cava. Compression may be caused by benign tumors, bronchogenic lung cancer, enlarged lymph, etc.


Superior vena cava syndrome (SVCS), is a group of symptoms caused by obstruction of the superior vena cava ("SVC"), a short, wide vessel carrying circulating blood into the heart. The majority of cases are caused by malignant tumors within the mediastinum, most commonly lung cancer and non-Hodgkin's lymphoma, directly compressing or invading the SVC wall. Non-malignant causes are increasing in prevalence due to expanding use of intravascular devices (such as permanent central venous catheters and leads for pacemakers and defibrillators), which can result in thrombosis. Other non-malignant causes include benign mediastinal tumors, aortic aneurysm, infections, and fibrosing mediastinitis.

Slovakian Stefan Zoleik underwent a five-hour procedure to remove the whopping 13lbs tumour from underneath his face and is now recovering after putting up with it for a decade.

Health problems stopped him from getting surgery for the lump, that was growing from ear to ear like a giant beard made out of skin, but Stefan is now looking forward to a normal life at last.

After having his bandages removed for the first time, the retired plumber said: "I'm really looking forward to getting back to normal.

"I'll finally be able to go for a beer with friends without people turning their heads and pointing at me.

"My family were really surprised how well I looked when they came to visit me in the hospital after the operation."

Stefan's growth was the result of a rare condition known as Madelung disease, which causes fatty tumours to grow around the neck.

The size of the lump led Stefan to stay indoors as people would turn and look at him wherever he went – something he couldn't do himself as he would struggle to move his head properly.

The dad-of-two added: "Gradually, my face began to swell. I had to stay at home for years.

"Everybody was turning and looking at me. It was very unpleasant. It also bothered me all the time when I moved my head."

Despite his joy at having the growth removed, it may yet return.

Igor Homola, the surgeon who removed the tumour, warned: "The tumour can not be removed completely and may even return."
